Background: Breast Surgery is becoming more and common. Patients are often middle-aged women with high public awareness around issues of breast cancer. The numbers increasing. surgery an exceedingly common procedure associated increased incidence acute chronic pain. Regional anesthesia techniques may improve postoperative analgesia for patients undergoing surgery. Objective: To assess the efficacy safety ultrasound-guided serratus anterior plane block (SAPB) compared thoracic epidural (TEA) surgery. methods: This a prospective randomized clinical study done at Aswan University Hospitals. present was conducted on sixty female ASA I-II, their ages ranged from 20-50 years scheduled unilateral were allocated randomly into three groups (n=20 each) according to type regional administrated. Results: There no significant difference between studied in age, body weight, height BMI. VAS pain scores throughout first 24 hrs showed that there highly statistically decrease immediate till 6 SAPB group (VAS C less than A B, P-value < 0.001). As regard side effects, complications recorded such as pneumothorax, vascular puncture, or local anesthetic toxicity. Conclusion: Serratusanterior blockmaintained hemodynamic stability TE it produced low total morphine consumption early period after cancer These advantages, suggest usefulness especially outpatient
